摘要： 目的：探讨母亲孕前体质指数（Body Mass Index, BMI）与母亲外周血及胎儿脐带血中炎症因子水平的关系。方法：选取2014年9月至2015年1月在哈尔滨医科大学附属第二医院住院分娩的123例孕妇及其新生儿为研究对象。收集孕妇孕前体重、身高，新生儿性别、出生体重等基本信息，以及孕妇外周血和胎儿脐带血。检测血中肿瘤坏死因子α（TNF-α）、白细胞介素6（IL-6）、超敏C反应蛋白（hsCRP）等炎症因子水平。采用线性回归分析母亲孕前BMI与其外周血及脐带血中炎症因子水平的关系。结果：在校正可能的混杂因素之后，母亲孕前BMI与其外周血和脐带血中TNF- α、IL-6及hsCRP的浓度显著相关。孕前BMI每增加1 kg/m2，脐带血中TNF- α浓度增加6.3（95%置信区间CI：0.8-11.9）pg/ml；IL-6浓度增加3.9（95% CI：0.3-7.5）pg/ml；hsCRP浓度增加0.03（95% CI：0.001-0.05）ug/ml。结论：母亲孕前BMI不仅与自身炎症水平有关，而且与胎儿体内的炎症水平有关，从而可能影响子代成年后代谢性疾病的发生风险。

Abstract： Objective: To explore the association of maternal pre-pregnant body mass index (BMI) with inflammation markers in maternal and umbilical cord blood. Methods: The 123 eligible pregnant women and their newborns were recruited from the Second Affiliated Hospital of Harbin Medical University from September, 2014 to January, 2015. Maternal pre-pregnant body weight and height and newborns' sex, birthweight were collected using questionnaire. Maternal peripheral blood and umbilical cord blood were collected to detect the concentration of TNF-α, IL-6, and hsCRP. Linear regression model was used to analyze the association between maternal BMI and inflammation in maternal and fetal blood. Results: After adjustment for confounding factors, maternal BMI was significantly associated with the concentration of inflammation factors in maternal and fetal blood. For every 1 kg/m2 increase in maternal BMI, fetal TNF-α level increased by 6.3 pg/ml (95% CI: 0.8-11.9), IL-6 increased by 3.9 pg/ml (95% CI：0.3-7.5), and hsCRP increased by 0.03 ug/ml (95% CI: 0.001-0.05). Conclusion: Maternal pre-pregnant BMI was significantly associated with inflammation in maternal and fetal blood, which may increase the risk of metabolic diseases in offspring in adulthood.
